Tier 1: Meta-Analysis of Human Trials

This is a research technique that combines and analyzes data from multiple individual studies to reach a single, stronger conclusion.

It increases statistical relevance, clarifies conflicting results, and allows researchers to evaluate whether an effect is consistent across different populations.

This is the strongest form of evidence that an herb has the functional effects and benefits claimed.

Tier 2: Randomized, Double-Blind, Placebo-Controlled, Human Trials

This is the gold standard study design for the best evidence in a single trial.

Randomized means participants are put into groups by chance.

Double-blind means neither the participants nor the researchers know who gets the real herb(s) and who gets the fake ones.

Placebo-controlled means the trial compares the herb(s) against a fake or inactive form of the herb.

And these are all done in humans (not animals or in vitro).

Tier 3: Other Human Trials

This tier covers human herbal research that stops short of tier 2. They are real studies in real people, but with looser controls that leave more room for bias. It includes open-label, non-randomized, observational, and case series.

Open-label means everyone knew who was getting what herb, so the placebo effect and expectation can color the results.

Non-randomized means participants were sorted into groups by something other than chance, which can stack the groups unevenly.

Observational studies watch what happens without assigning any treatment, so they can spot associations but can't prove one thing caused another.

Case series simply track a group of people who all tried the same herb, documenting what followed.

Tier 4: Traditional Wisdom From Established Herbal Systems

This includes insights from Ayurvedic Herbalism, Traditional Chinese Herbalism, Taoist Herbalism, Western Herbalism, and Native American Herbalism.

All of these traditions have thousands of years of exploring herbal remedies with patients, recording the effects and benefits, and updating their records over time.

There are many confounding factors not taken into account with this type of evidence, but it is still incredibly useful because the sheer scale of human use behind this evidence (across cultures and centuries) far exceeds what any single trial can capture. Where traditional wisdom and modern human trials meet is my favorite form of evidence.

Tier 5: Animal Studies (In Vivo)

Animal studies using herbal remedies involve testing extracts on laboratory or domestic species to evaluate pharmacological effects, toxicity limits, and therapeutic potential.

These in vivo experiments help determine how botanical compounds affect biological systems before human application.

Because human bodies are different from other animal bodies, we can't glean too much proof from these studies but they are still useful in helping frame our understanding for further research.

Tier 6: In Vitro Studies

In vitro studies using herbal remedies test extracts on isolated cells, tissues, or enzymes outside a living organism, focusing on cellular responses, mechanisms of action, and safety limits.

These controlled laboratory experiments mainly examine specific bioactive compounds, cytotoxicity, anti-inflammatory effects, and antioxidant capacity.

Even though this is the lowest tier of herbal evidence, we can gain important insights that are very difficult to study in human bodies such as how bioactive compounds interact with specific components inside of cells.
